When you work with HIGHPOWER, you will gain access to a team of experienced and knowledgeable professionals who will utilize our tried-and-true quality management system throughout all phases of your project, including the sample submission form. All devices or test samples submitted to HIGHPOWER for testing should be accompanied by a completed sample submission form document. Samples submitted without a completed sample submission form will remain on hold until one is obtained. This form allows us to quickly document information about the samples obtained along with the required storage, intended use and safety information.

A completed sample submission form is necessary prior to the start of your project to verify any discrepancies, as the samples received in by HIGHOPWER will also be logged in utilizing a receiving inspection form. From the time we receive your sample documentation, we begin recording data regarding your project. Please note that our submission paperwork requires you to be as descriptive as possible about your product or sample.

Filling Out HIGHPOWER Sample Documentation / Sample Submission Form

Information is key when submitting a sample for the proper validation testing. In addition to details about your business and how to contact you, it’s also important that you provide us relevant information regarding the samples, including lot numbers, serial numbers, bill of materials, and quantity of samples. The SSF has several fields for listing the different parts you want tested as well as a section to indicate sample storage requirements and sample disposition after completion.

If your device has special instructions, including specific sterilization parameters and special handling guidelines, please include them where indicated.
